Abstract
Pure spinal epidural cavernomas are very rare pathologies. They constitute only 4% of allspinal epidural tumors. Their radiologic appearance may mimic many other spinal epiduralpathologies such as schwannoma, meningioma, lymphoma, Ewing's sarcoma, chordoma,ependymoma, spinal angiolipoma, perineural cysts and disc herniation. Complete surgicalresection is the preferred treatment. But unfortunately they usually bleed heavily duringsurgery that makes complete surgical resection difficult.Here we report a case of sacral pure spinal epidural cavernoma mimicking S1 rootschwannoma. A 37 years old female patient presented with low back pain and radiculopathicsymptoms. After MRI scans, the lesion was prediagnosed as S1 root schwannoma. Patientwas operated and pathologic examination revealed that the lesion was a cavernoma. Aftersurgical resection the patient' s symptoms completely disappeared and did not recur for 15months.

Spinal epidural kavernomlar nadir rastlanan patolojilerdir. Bütün spinal epidural tümörlerin%4' ünü oluştururlar. Radyolojik görüntüleri schwannom, meningiom, lenfoma, Ewingsarkomu, kordoma, ependimom, spinal anjiyolipom, perinöral kist ve disk herinisiylekarışabilir. Tedavide total cerrahi rezeksiyon önerilmektedir ancak aşırı kanama nedeniyle bubaşarılamayabilir. Burada 37 yaşında bel ve sol bacak ağrısı şikayetiyle başvuran bir bayanhasta sunuldu. MR tetkikleri sonucu lezyonun S1 kök schwannomu olduğu düşünüldü.Cerrahi rezeksiyon sonrası patoloji tanısı kavernom olarak raporlandı ve şikayetleri tamamenkayboldu ve 15 aylık takibinde tekrarlamadı.
