Abstract

OBJECTIVE: The present study aims to assess the consistency between original histopathologic diagnosis and second opinion in referred cases to our institution.STUDY DISGN: We reviewed gynecopathologic reports from 199 referred patients. Each case was evaluated by comparing both the original and consulted pathology reports. A discrepancy was defined major if it led to treatment or prognosis alteration.RESULTS: In 203 lesions from 199 referred patients, 167 (82 %) showed no discrepancy. The major discrepancy rate was 8 % and the minor discrepancy rate was 10 %. Our study revealed endometrial and cervical lesions were significantly more likely to have change in diagnosis.CONCLUSIONS: Second opinion gynecopathology can result in major therapeutic and prognostic modifications for patients sent to large referral hospitals.

Özet

AMAÇ: Çalışmamızın amacı, başka bir sağlık kurumunda tanı aldıktan sonra fakültemize gönderilen ve patoloji preparatları tekrar değerlendirilen jinekopatoloji olgularında ilk konulan patoloji tanısı ile konsültasyon sonrası konulan tanıyı karşılaştırmak ve iki tanı arasındaki uyumu araştırmaktır.MATERYAL ve METOD: Konsülte edilen 199 jinekolojik olguya ait patoloji raporları gözden geçirildi. Her olguda orijinal rapor ve konsültasyon raporu karşılaştırıldı. Tedavide veya prognozda değişikliğe yol açan tanı uyumsuzlukları majör uyumsuzluk olarak değerlendirildi.BULGULAR: Yüzdoksan dokuz olguya ait 203 lezyondan 167'sinde (% 82) tanılar uyumlu iken 19 (% 10) lezyonda minör uyumsuzluk, 17 (% 8) lezyonda majör uyumsuzluk saptandı. Çalışmamızda majör uyumsuzluk en çok endo-metriyum ve servikse ait lezyonlarda görüldü.SONUÇ: Jinekopatolojide ikinci kez inceleme büyük merkezlere gönderilen hastalarda majör tedavi ve prognoz değişikliklerine yol açabilmektedir.
