Abstract

Regulation of blood glucose is very important during peripartum period since maternal hyperglycemia in this period increases risk of neonatal hypoglycemia. Generally, metabolic demands are very small in latent phase of parturition. In active phase, metabolic demands increase and insulin requirement decreases. Different insulin treatment protocoles can be used for glucose control during parturition. Alternate liquid strategy is preferred in patients with gestational diabetes during intrapartum period since it decreases insulin requirement. In patients with pregestational diabetes, intravenous insulin infusion or subcutaneous insulin treatment can be used during peripartum period. In the postpartum period, with the removal of placenta, insulin resistance improves quickly and insulin requirement decrease in patients with pregestational diabetes. Patients with gestational diabetes return quickly to pregestational glycemic state and generally do not need insulin treatment. However, in some patients with gestatonal diabetes, hyperglycemia may continue in the postpartum period and treatment can be required. American Diabetes Association and The American College of Obstetricians and Gynecologists recommend screening all patients with gestational diabetes with 75 gr 2 hour oral glucose tolerance test in the postpartum 4-12th weeks in order to detect diabetes, impaired fasting glucose and impaired glucose tolerance.
