Abstract

Objective: Minor head injury is a common reason for admission of children in emergency department. Computed tomography (CT) is frequently used in managing traumatic brain injuries in children. The aim of this study was to present usefulness CT in minor head injury.Material and Methods: We retrospectively evaluated cranial CT examinations of children admitted to the emergency department of our hospital with minor head injury, defined by Glasgow Coma Scale (GCS) scores of 14-15. Patients were assessed according to their age groups, mechanism of injury, symptoms and physical examination findings, and radiological investigation results at their first referral to the emergency service. Abnormal imaging study results were defined as skull fracture, epidural, subdural, subaracnoidal and/or parenchymal hemorrhage. Results: Three hundred and fourteen patients were enrolled in the study. Study population consisted of 199 (63.4 %) boys and 115 (36.6 %) girls. Nineteen patients (6 %) had pathological CT findings (skull fracture, n= 11, epidural hemorrhage, n= 1; subdural hemorrhage, n= 2; subaracnoidal hemorrhage, n= 3; parenchymal hemorrhage, n= 2). None of them required any neurosurgical intervention. Conclusion: The best way to manage children with minor head trauma is still a matter of debate. Children with blunt head trauma and initial GCS scores of 14 or 15 and normal CT scan results are at very low risk for subsequent traumatic findings on neuroimaging and extremely low risk of need for neurosurgical intervention. Our results suggest that minor head trauma does not always require CT. The radiation risks posed by CT scanning in children must be balanced by its benefits

Özet

Amaç: Minör kafa travması acil servise çocukların en sık başvurma nedenlerindendir. Bilgisayarlı tomografi (BT) çocuklarda travmatik beyin yaralanması yönetiminde sık kullanılır. Bu çalışmanın amacı minör kafa travmasında BT yararlılığını sunmaktı.Gereç ve Yöntemler: Minör kafa travması [Glaskow koma skalası (GCS) skoru 14-15] ile hastanemiz acil servisine başvurup bilgisayarlı beyin tomografisi yapılan çocuklar retrospektif olarak değerlendirildi. Hastalar yaş gruplarına, travma nedenlerine, acil servise başvuru sırasındaki belirti ve bulgulara, radyolojik inceleme sonuçlarına göre değerlendirildi. Anormal görüntüleme sonuçları kafatasında kırık, epidural kanama, subdural kanama, subaraknoidal kanama ve parankimal kanama olarak tanımlandı.Bulgular: Üç yüz on dört hasta çalışmaya dâhil edildi. Çalışmamız % 63.4 (n=199) erkek, % 36.6 (n=115) kız hastadan oluşmaktaydı. On dokuz hastada (% 6) patolojik BT bulguları mevcut olup (11 kafatasında kırık, 1 epidural kanama, 2 subdural kanama, 3 subaraknoidal kanama, 2 parankimal kanama), hiçbirisine cerrahi tedavi gerekmedi. Sonuçlarımız minör kafa travmasının her zaman BT gerektirmediğini önermektedir. Çocuklardaki BT taramanın oluşturduğu radyasyon riskleri ile yararları dengeli olmalıdır
