Abstract

Serum Monocyte Chemoattractant Protein-1 (MCP-1) is a very strong chemotactic factor for monocytes. Recent studies show that oxidized LDL stimulates the production of MCP-1 by the cells of the vessel wall. Therefore, maybe the MCP-1 is the possible moleculer connection between oxidized LDL and the accumulation of foam cells in the vessel wall and atherosclerosis. Cardiovascular and cerebrovascular mortality due to accelerated atherosclerosis is obviously high in Chronic hemodialysis patients. Also elevated serum concentrations of various cytokines including MCP-1 and chemokines have been shown in chronic hemodialysis patients. However, there are not enough studies to show the relation between the atherosclerosis and serum MCP-1 level. The aim of this study is to evaluate the relation between fibrinogen, MCP-1 and atherosclerosis and the severity of chronic renal failure. This study enrolled 40 chronic renal failure patients (GFR< 15, GFR 15-29, GFR 30-59, GFR 60-89) who registered to the outpatient nephrology service of Izmir Ataturk Training Hospital, and 10 healty subjects. (GFR 90)The subjects with. GFR 90 were considered as a control group. Uric acid, BUN, creatinin, triglyceride, total cholesterol, HDL, LDL, VLDL, ApoA1, ApoB, MCP-1 and fibrinogen were assessed in all participants. All participants underwent ultrasonic measurements of carotid artery intima-media thickness to investigate atherosclerosis. The relation between carotid IMT levels and GFR were statistically significant between the patients and the control gruop. Also the fibrinogen levels significantly higher in patients than the control group. But There were no significant association between serum MCP-1 levels and carotid IMT measurement and fibrinogen levels. Our results revealed that high plasma fibrinogen levels in chronic renal failure patients is an importat risk factor for cardiovascular disease. In contradistinction to expectations the MCP-1 levels were low in our study we thougt because of our patients were using drugs lowering serum MCP-1 levels to avoid chronic renal failure complications.

Beklenenin aksine serum MCP-I düzeylerinde saptanan düsüklügün hastalarımızın kronik renal hastalık komplikasyonlarını önlemek için kullandıkları ve MCP-I düzeylerini etkiledigi bilinen ilaçlara baglı olabilecegini düsündük.
