Abstract

A myeloma case with multiple paraproteinemia and fatal progress. Multiple myeloma (MM) is a lethal plasma cell proliferation that involves the bone marrow and extra-osseous tissues in a multi-focal fashion. Characteristically, myeloma incidence increases with age. Like other plasma cell dyscrasias, myeloma is associated with the expansion of a single clone of immunoglobulin (Ig)-secreting cells that results in the secretion of a unique homogenous Ig product (M component). However, about 1 % of the myeloma patients have more than one such protein detectable in their sera. Multiclonal gammopaties associated with MM may result either from a neoplastic transformation of a cell clone undergoing Ig class switching or from independent transforming events yielding proliferation of unrelated plasma cell clones. We report a patient, diagnosed as having MM stage IIIB,.whose serum presented IgA-K paraproteinemia and have two well defined homogenous K molecules. He had renal failure, hypercalcemia and hyperviscosity sendrome. After two.plasma exchange and chemotherapy he lived for a short time and died of bleeding and infection. The clinical evolution of MM cases with biclonal components was described as indistinguishable from MM associated with a single monoclonal protein though some investigators found similar to our case a shortened survival for biclonal gammopathies.
