Abstract

Hemiprosthesis or total prosthesis application is the preferred treatment method for the elderly patients who have collum femoris fracture. Daily activity, life expectancy, acetabular degeneration and cost should be taken into consideration while choosing prosthesis. Eighty-one patients who had collum femoris fracture and were treated with unipolar and bipolar prosthesis in 1996-2002 at Haydarpaşa Numune Education and Research Hospital 1st Orthopedics and Traumatology Clinic have been reached. It was learned that 18 patients (10 unipolar, 8 bipolar) had died. For 63 patients (33 bipolar, 30 unipolar) the mean age was 73, the mean follow-up period was 36.8 (10-84) months. When we evaluated these patients by the modified Merle D'aubigne score, we found 54.3% excellent and good results for the bipolar group and 53.3% for the unipolar group. Acetabular score was 95.6 for the bipolar group and 93.6 for the unipolar group. Bipolar prosthesis is nearly two times expensive than the unipolar prosthesis. There were no differences between evaluated groups for the functional results, complications, acetabular erosion and mortality. In this occassion, the bipolar prosthesis is not cost effective for the patient whose life expectancy is very low.

Özet

Kollum femoris kırığı geçiren yaşlı hastalarda parsiyel veya total protez uygulaması seçkin tedavi yöntemidir. Protez seçimi yapılırken hastanın günlük aktivitesi, yaşam beklentisi, asetabulumun durumu ve protez maliyeti göz önünde tutulur. 1996-2002 yılları arasında Haydarpaşa Numune Eğitim ve Araştırma Hastanesi 1. Ortopedi ve Travmatoloji Kliniği'nde kollum femoris kırığı nedeniyle unipolar veya bipolar protez uygulanan 81 hastaya ulaşıldı. On sekiz hastanın vefat ettiği öğrenildi (10 unipolar, 8 bipolar).
