Abstract

Evaluation of AIDS Cases Between 1988 and 2004 with Respect to Pulmonary Involvement and Clinical Course Despite not a common health problem in Turkey yet as it is in the world, new cases of Acquired Immunodeficiency Syndrome (AIDS) and Human Immunodeficiency Virus (HIV) carriers are recognized each year. Especially when seen with tuberculosis (TB), the course of the disease gains acceleration and extrapulmonary TB forms are more common. A total of 19 HIV positive cases, two of them being of foreign born (13 male, 6 female) hospitalized at Heybeliada Chest Disease Hospital between 1988 and 2004 were evaluated retrospectively. Five of them were drug abusers, 3 homosexuals and drug abusers, 2 homosexuals, 2 prostitutes, 2 were HIV carriers. There was history of blood transfusion in one and no identifiable risk factor in 3 cases. TB was diagnosed in 16 cases (84%), lung cancer and pneumonia each in one. No lung pathology was detected in one case. Out of 14 TB cases, pulmonary involvement was cavitary in 11 (78.6%) and interstitial in 3 patients accompanied by hilar and mediastinal lymphadenopathy (LAP) in 2 and servical LAP in one. In a pulmonary TB patient, there were also skin lesions consistent with Kaposi sarcoma. Out of two cases with only extrapulmonary involvement, one had servical LAP and the other miliary TB. The diagnosis of TB was established bacteri-ologically in 14 (87.5%) and histopathologically in 2 cases. TB cases should carefully be assessed with respect to risk factors for AIDS and HIV positivity should be sought especially when there is extrapulmonary TB and radiologic findings of TB are atypical.
