Abstract

Thyroidectomy is usually performed durind laryngeal surgery at most of the clinics. However, there is no concencus about thyroidectomy at the treatment of larynx cancers. We examined the effect of larengeal surgery, thyroidectomy and postoperative radiotreatment over thyroid gland functions on 36 patients with larynx squamous cell carcinoma. We determined hypothyroidism at 6 patients out of 36 (16.7%) by examining free T3, free T4 and TSH levels postoperatively. All these 6 pati-ents had total laryngectomy and 3 of them had hemithyroidectomy isthmectomy and postoperative radiotheraphy in addition to total laryngectomy. 2 patients had only postoperative radiotheraphy and 1 patient had hemityroidectomy and ›sthmectomy in addition to larynx surgery. The was no correlation statistically between hemityroidectomy isthmectomy in addition to laryngeal surgery and hypotyroidism. There was no correlation between only postoperative radiotheraphy and hypothyroidism but we found a statistically significant correlation between hemithyroidectomy isthmectomy and postoperative radiotheraphy in addition to laryngeal surgery with hypothyroidism. However, one intact lobe of the thyroid remains, sufficient compensation may not be possible everytime because of dissection of the vessels when combined with postoperative radiotheraphy which causes fibrozis and thyroiditis of the gland. It may be possible to avoid hypothyroidism complications by routinely examining thyroid hormone levels at the postoperative period especially if the patient had additional treatment modalities suchs as radiotheraphy and hemithyroidectomy.

Özet

Bircok klinikte hemitroidektomi larenjektomi operasyonunda rutin olarak uygulanmıştır. Fakat larengeal kanserlerde tiroid cerrahisi icin rutin bir goruş yoktur. Bu calı şmada Haydarpaşa Numune Eitim ve Araştırma Hastanesi Kulak Burun Boaz Klinii’nde fiubat 2002 ve fiubat 2007 tarihleri arasında skuamoz hucreli karsinom tanı sı ile larenjektomi operasyonu uygulanmış tiroid bezi fonksiyonları yonunden salıklı 36 olguyu inceleyerek uygulanan cerrahinin ve postoperatif radyoterapinin tiroid bezi fonksiyonları uzerindeki etkisi araştırdık. Hastaları n preoperatif tiroid hormonu seviyeleri incelenmesinde hicbir hastada hipertiroidi veya hipotiroidi saptanmamı ştır. Salıklı 36 larenks skuamoz hucreli karsinom olgusunun postopereatif serum sT3, sT4 ve TSH duzeyleri incelendiinde 36 olgunun 6’sında hipotirodi bulunmuştur (%16,7). Hipotiroidi bulunan 6 olgunun hepsine total larenjektomi uygulanmıştı.
