Abstract

Purpose: To evaluate the diagnostic value of MDCT colonography, a new noninvasive method, in identifying colorectal polyps in adult patients with a high risk for colonic cancer. Method: From February 2008 to January 2009 a total of 22 patients (11 men, 11 women) with a mean age of 60.63 years (range 43-81 years) were examined. Inclusion criteria were age over 50 years, history of rectal bleeding, occult blood in feces, iron deficiency anemia, history of polyps, familial colonic cancer, previous surgery due to colorectal cancer and incomplete conventional colonography due to technical difficulties. MDCT colonography was performed with a 40 slice scanner (Somatom Sensation Siemens 40 syngo CT 2007S). After completing MDCT colonography, conventional colonoscopy was performed on the same day. Pathological findings in MDCT colonography were defined according to the localization of the pathology in the colon and reported on the same day independently from conventional colonography examination. Results: Of the 22 patients, conventional colonoscopy was incomplete in 3 patients due to obstructive colonic cancer in two and colonic tortuosity in one. Colonic pathologies were detected in 11 patient(%50). Of the 30 polipoid lesions which were detected by conventional colonography, 25 lesions were al- so detected by MDCT colonography. Retrospective analysis of the data shows that 5 of the 30 polipoid lesions were missed in MDCT colonography because of residual feces and mucosal folds. Sensitivity and specificity of MDCT colonography were 83.33% and 83.33%, respectively. Positive predictive value was 92.59% but negative predictive value was 66.67%. The sensitivity of the technique for polipoid lesions smaller than 5 mm, between 6-9 mm and over 10 mm was found to be 72.72%, 84.61% and 100%, respectively. Conclusion: MDCT colonography is a noninva- sive diagnostic method. Compared to conventional colonography which is an invasive method, MDCT colonography has reasonable sensitivity and specificity values in the diagnosis of the colonic polyps. Because of its noninvasive nature it may be accepted as an alternative to conventional colonography.
