Özet

Amaç: Çalışmamızda; perianal cerrahidehiperbarik bupivakain ve hiperbariklevobupivakainin saddlespinal bloközelliklerinin ve hemodinamik etkilerininkarşılaştırılması amaçlanmıştır.Gereç-Yöntem:Etik kurul onayı ve hastalarınyazılı onayı alındıktan sonra, 18-50 yaş, ASAI-II, elektifperianal cerrahi planlanan 60 hastaçalışmaya dahil edildi. Hastalar randomizeedilerek grup B; %5 hiperbarikbupivakain7.5mg veya grup L; %5 levobupivakain 7.5 mgşeklinde iki eşit gruba ayrıldı.Saddlespinal anestezi sonrasında duyusal, motor blok seviye ve süresi kaydedildi. Kalphızı (KH), ortalama arter basıncı (OAB) çalışmasüresince; bazal, intratekalenjeksiyonsonrası 2.5, 5, 7.5, 10, 12.5, 15. dakikave takibinde 5 dakika aralıklarla ölçüldü.OABdebazal değere göre; %20 düşüşhipotansiyon olarak değerlendirildi ve efedrin5 mg (İV) uygulandı. KH 50 atım/dak. altınadüştüğünde atropin 0.5 mg (i.v.) uygulandı.Bulgular: Gruplar arasında demografiközellikler benzerdi. OAB ve KH ölçümlerinde gruplar arasında anlamlı fark yoktu.3 hastada(grup Lde; 2, Grup Bde; 1) efedrin ihtiyacıoldu. Duyusal ve motor blok süresi grupLde, grup Bden daha kısaydı.Sonuç: Hiperbarikbupivakain ve hiperbariklevobupivakain her ikisi de hemodinamikstabiliteleri ile perianal cerrahide saddlespinalanestezide uygundur.
