Abstract

Glucose-6-phosphate dehyrogenase (G6PD) deficiency is the most common hereditary enzyme deficiency. Major clinical manifestations are drug-induced hemolytic anemia and neonatal jaundice. A small portion of GePD deficient individuals have chronic non-spherocytic anemia. Favism is a severe acute hemolytic anemia which occurs in 20 % of glucose-6-phosphate deficient subjects after ingestion of fava beans. A 10-mont-old male infant was admitted to the outpatient clinics with pallor, vomiting, jaundice, pink urine. He was pale, icteric and had hepatosplenomegaly on physical examination. The laboratory findings revealed hemolytic anemia. The patient had a history of fava bean ingestion and his glucose-6-phosphate dehydrogenase enzyme activity was 3,05 U/gHb (normal: 5,5-14,4 U/gHb). We intend to present this case because his picture is example of favism which is rarely seen in infancy.

Özet

Glukoz-6-fosfat dehidrogenaz eksikliği en sık görülen kalıtsal enzim eksikliğidir. Klinikte ilaca bağlı hemolitik anemi, yenidoğan sarılığı ve kronik nonsferositik anemiye sebep olur. Favizm, glukoz-6-fosfat dehidrogenaz eksikliği bulunan hastaların %20'sinde bakla alımı sonrası görülen ağır hemolitik anemi tablosudur. On aylık erkek hasta kliniğimize solukluk, kusma, sarılık, idrarda pembelik şikayetleriyle başvurdu.
