Abstract

K awasaki disease is a vasculitis characterized by fever persisting at least 5 days and the presence of polymorphous exanthema, conjunctiva! injection, cervical lymphadenopathy, changes of the oral cavity and the extremities. Incomplete Kawasaki is characterized by fever and the presence of less than four of the principle features. This study was conducted in the Department of Pediatrics at Zeynep Kamil Hospital, with 15 patients between 1994 and 2003. The mean age of the patients was 46.9 ± 24.5 months and the male/female ratio was 1/1.1. Fever and changes in extremities were present in all patients. Ninety-three percent of the patients showed changes of the oral cavity, 60 % bilateral conjunctival injection, 86 % cervical lymphadenopathy and 86 % polymorphous exanthema. Sixteen percent of the twelve patients with complete Kawasaki disease showed coronary artery dilatation and another 16 %, revealed mild pericardial effusion. Three of the fifteen patients (20 %) were diagnosed as incomplete Kawasaki disease. Mild mitral insufficiency was observed in one of these patients. Accompanying findings in our Kawasaki cases were aseptic meningitis, hydrops of the gallbladder, gastroenteritis, sterile pyuria, pneumonia and arthraigia. Eleven patients were treated with intravenous immunoglobulin and acetylsalicylic acid. Although 25% of the patients in the non-treated group, showed coronary artery pathology only 9% of the patients treated with immunoglobin revealed coronary artery pathology. It is essential to remember Kawasaki disease in the differential diagnosis of fever persisting longer than five days. Early diagnosis and treatment is important to prevent the coronary artery complications.
