Abstract
True malignant mixed tumor of the salivary gland origin is exteremly rare. We presented true malignant mixed tumor which arisen from benign pleomorfic adenoma in this report. A 83-year-old female patient came our clinic with complaints of decreasing salivary excreation and rapidly growing mass at the right parotid region since last year, after a long period of time without any change in size. At the operation tumor excision and total parotidectomy were performed. At the pathological examination, benign pleomorfic adenom indifferansive carcinoma and high grade sarcomatous regions were present. Vimentin and keratin was found pozitive at the immunohisto chemical studies. It is assumed from these clinical and histological findings that the tumor was transformed to true malignant mixed tumor (carcinosarcom) from a pre-existing benign pleomorfic adenoma.
Özet
Tükrük bezlerinin gerçek malign mikst tümörü çok nadir görülür. Bu bildiride benign pleomorfik adenom zemininden gelişmiş gerçek malign mikst tümör olgusunu sunduk. 83 yaşında kadın hasta, sağ parotis lojunda, 40 yıl önce ortaya çıkıp, son bir yıldır hızla büyüyen kitle ve tükrük salgısında azalma yakınmasıyla bize başvurdu. Yapılan ameliyatla tümör, parotis bezinin tamamı ile birlikte çıkarıldı. Patolojik incelemede, indiferansiye karsinom alanları, yüksek grade sarkomatöz alanlar ve benign pleomorfik adenom alanları saptandı. İmmünohistokimyasal çalışma sonucu, vimentin (+), keratin (+) olarak tesbit edildi. Bu klinik ve patolojik bulgular ışığında, tümörün benign pleomorfik adenom zemininden dönüşmüş true malign mikst tümör (karsinosarkom) olduğuna karar verildi.
