Abstract

Objective: The aim of this study was to determine the risk factors and the effect of prophylactic antibiotic use on surgical site infection after elective operations in neurosurgery and orthopedic departments. Methods: Prospectively 154 patient evaluated in neurosurgery and orthopedic departments at the Medical Faculty of Marmara University Hospital. All data collected by the method of face to face interview according to “Patient Follow-up Form”. The patients observed fort the sing and symptoms of infection and prophylactic antibiotic use, from the beginning to the end of hospitalization. After discharge the patients were asked about the sing and symptoms of infection by telephone interview. Results: At the end of the study surgical site infection identified in thirteen patients: Pseudomonas aeruginosa and Enterococcus fecalis were responsible for surgical site infections in two patients. Multivariate Logistic Regression Analysis of surgical site infection risk factors showed that urinary catheter use increase infection risk 5.8 times (Odds ratio;5.80, P value=0.031, %95 Confidence intervals; 1.17-28.61). Other risk factors (duration of hospitalization, prosthesis implantation, duration of operation, body mass index) were not statistically related with surgical site infection. Conclusion: In order to prevent the development of surgical site infection it is important to determine the risk factors and not to apply invasive procedures.

Özet

Amaç: Bu çalışmanın amacı, ortopedi ve beyin cerrahisi ünitelerimize elektif cerrahi girişim uygulanan hastalara Profilaktik Antibiyotik (PA) uygulamasının Cerrahi Alan Enfeksiyonu (CAE)’na etkisi araştırmak ve CAE ile ilişkili risk faktörlerini belirlemektir. Yöntemler: Marmara Üniversitesi Tıp Fakültesi Hastanesi ortopedi ve beyin cerrahisi ünitelerinde, 154 hasta prospektif olarak incelendi. Veriler araştırmacı tarafından hazırlanan “Hasta İzlem Formu” aracılığı ile yüz yüze görüşme yöntemi kullanılarak toplandı. Hastalar cerrahi girişimden taburcu olana kadar her gün CAE’nu ve aldığı antibiyotik yönünden izlendi. Taburcu olduktan sonra birinci, ikinci, üçüncü ve altıncı aylarda telefon anketi uygulanarak CAE semptomları soruldu. Bulgular: Çalışmanın sonunda 13 hastada CAE geliştiği belirlendi. CAE’nu gelişen 13 hastadan birinde Pseudomonas aeruginosa, diğerinde Enterococcus fecalis ürediği tespit edildi. CAE Gelişimini Etkileyen Risk Faktörlerinin Çok değişkenli lojistik regresyon analizinde idrar sondasının enfeksiyon riskini 5.8 kat (Odds oranı; 5.80, P=0.031, %95 güven aralığı; 1.17-28.61) artırdığı bulundu. Diğer risk faktörlerinin (hastanede kalış süresi, protez takılması, operasyon süresi, vücut kütle indeksi) CAE ile istatistiksel ilişkisi gösterilemedi.Sonuç: Hastada CAE gelişimini etkileyen risk faktörlerinin belirlenmesi, çok gerekmedikçe invaziv girişimlerin uygulanmaması CAE gelişmesini önleyebilir.
