Abstract

Background: This study aims to analyze the data regardingcases diagnosed with cor triatriatum sinistrum at a singlepediatric center, highlighting symptoms on presentation ofthe anomaly, mode of diagnosis, age at the time of diagnosis, any other accompanying cardiac defects, and outcomes. Methods: Records of 21 patients (13 females, 8 males;mean age 32.4 month; range 7 days to 57 years) diagnosedwith cor triatriatum sinistrum between January 1997and January 2012 at the pediatric cardiology clinic wereanalyzed retrospectively. Medical records includingdemographic data, clinical progress, diagnostic approach, and surgical interventions were reviewed. Results: The majority of patients (81%) had accompanyingcardiac defects, atrial septal defect being the most frequent(33%). Restrictive pattern was detected in five patients.Seven patients (33%) died after diagnosis. We resectthe fibromuscular diaphragm in 11 of 21 patients. Onepatient died after operation. Most common presentingsymptoms were lower respiratory tract infections, dyspnea, and early fatigue. Sixteen patients were diagnosed withechocardiography. Conclusion: Despite the diversity in clinical findingsat presentation and accompanying cardiac lesions, cortriatriatum sinistrum is a treatable defect with satisfyingtreatment outcomes.

Özet

Amaç: Bu çalışmada tek bir pediatrik merkezde kortriatriatum sinistrum tanılı olgularla ilgili bilgiler analizedildi, anomalinin hangi semptomlarla ortaya çıkabileceği,tanı yöntemi, tanı anındaki yaş, eşlik edebilen diğerkardiyak defektler ve sonuçlar belirtildi. Çalışmaplanı: Ocak 1997 - Ocak 2012 tarihleri arasındapediatrik kardiyoloji kliniğinde kor triatriatum sinistrumtanısı konulmuş 21 hastanın (13 kadın, 8 erkek; ort.yaş 32.4 ay; dağılım 7 gün-57 yıl) kayıtları retrospektifolarak incelendi. Demografik verileri, klinik seyri, tanısal yaklaşımı ve cerrahi girişimleri içeren tıbbikayıtlar değerlendirildi. Bul gu lar: Hastaların çoğunda (%81) eşlik eden kardiyakdefekt vardı ve en sık olanı atriyal septal defekt idi(%33). Beş hastada restriktif patern saptandı. Yedi hasta(%33) tanı sonrasında kaybedildi. Yirmi bir hastanın onbirine fibromusküler diyaframın rezeksiyonu uygulandı.Ameliyat sonrası bir hasta kaybedildi. En yaygın başvurusemptomları alt solunum yolu enfeksiyonları, nefes darlığıve çabuk yorulma idi. On altı hastaya ekokardiyografi iletanı konuldu. Sonuç: Başvuru anındaki klinik bulguların değişkenliğive eşlik eden kardiyak lezyonlara rağmen, kor triatriatumsinistrum tedavi edilebilir ve tedavi sonuçları tatmin ediciolan bir defekttir.
