Abstract

Objective: In this retrospective study, we shared our single-center experience aimed to evaluate the efficacy and safety of brentuximab vedotin (BV) treatment in relapsed/refractory (R/R) hodgkin lymphoma (HL) patients. Patients and Methods: Between July 2000 and November 2018 we analyzed a total of 18 R/R HL patients in our hematology clinic. There were 8 (44.5%) female, 10 (55.5%) male patients. Median age was 35.1 years (range, 22-58 years). The median time of follow-up was 43 months (range, 18-162 months). All patients received first line ABVD (Doxorubusin, Bleomycin, Vinblastine, Dakarbazine) treatment and autologous stem cell transplantation was applied. Results: Median time from diagnosis to first BV tretament is 36 range, (11-161) months. 16 patients received BV after OKHN and 2 patients after allogeneic transplant. BV completed 3 alıcycles of therapy in 12 patients, with a median of 5 cycles (range, 2-16). The number of patients reaching complete remission was 4 (22%) and partial remission 6(33%), stabl disease was 4 (22%), progressive disease 4 (22%). Relapse occurred in 2 (20%) of 10 patients who responded to BV treatment in the first 12 months and then median death time was 4 months (range, 3-5 months). The probability of oneyear- overall progression-free survival was %39. Conclusion: BV is a tolerable treatment option, seems providing pre-transplant disease control as well as significant improvement in post-transplant outcomes in R/R HL.

Özet

Amaç: Bu retrospektif çalışmada relaps/refrakter (R/R) hodgkin lenfoma (HL) hasta grubunda brentuksimab vedotin (BV) tedavisinin etkinlik ve güvenilirliğini göstermeyi amaçlayan tek merkez deneyimimizi paylaştık. Hastalar ve Yöntem: Temmuz 2000-Kasım 2018 tarihleri arasında Ankara Üniversitesi Hematoloji Bilim Dalı’nda takibi yapılan 18 R/R HL hastası dahil edildi. Hasta dosyaları retrospektif olarak değerlendirildi. Hastaların 10’u erkek, ortanca yaşı 35.1 (22-58), ortanca takip süresi 43 ay (18- 162) idi. Bütün hastalar ilk sıra ABVD (Doksorubusin, Bleomisin, Vinblastin, Dakarbazin) tedavisi aldı ve otolog kök hücre nakli (OKHN) uygulandı. Bulgular: Tanıdan ilk BV uygulamasına kadar geçen ortanca süre 36 (11-161) ay olup, 16 hasta OKHN, 2 hasta allojeneik nakil sonrası BV aldı. BV, 12 hastada 3 kür tedaviyi tamamlayarak, ortanca 5 kür verildi (2-16). BV alan 4 hastada tam yanıt (%22), 6 hastada parsiyel yanıt (%33), 4 hastada stabil hastalık (%22), 4’ünde ise progresyon (%22) görüldü. BV tedavisine yanıtlı 10 hastanın 2’sinde (%20) ilk 12 ayda nüks gelişti ve sonrası ortanca 4 ayda eksitus (3-5) oldu. BV ile tedavi yanıtı alınamayan 8 hasta farklı tedaviler aldı. BV sonrası ortanca takip süresi 9 ay (1-27), 1 yıllık progresyonsuz sağkalım %39’dur. Sonuç: BV R/R HL hasta grubunda nakil öncesi hastalık kontrolü sağlamada etkin olduğu gibi, nakil sonrası sonuçlarda da belirgin düzelme sağlayan, tolere edilebilir bir tedavi seçeneğidir
