Abstract

This case report aims to present that drugs administered during anesthesia may effect identical twins similarly and compares complications. Our patients, 59 year old identical twins, both had ejection fractions of 63% and stenotic lesions in left anterior descending (LAD), circumflex (Cx) second obtus and right coronary artery in coronary angiography. Coronary artery bypass grafting was planned and performed simultaneously. Both patients showed similar myocardial performance while weaning from bypass. Both cases had similar hemodynamic characteristics, activated clotting time (ACT), blood sample values and drug doses. As a result, identical twins have similar intraoperative findings and clinical outcomes, which may have important implications while planning same type of operations and preventing complications.

Özet

Bu olgu sunumunun amacı tek yumurta ikizlerinde anestezi sırasında uygulanan ilaçların etkileri veya uygulama sonucu meydana gelebilecek komplikasyonları karşılaştırmaktır. 59 yaşında tek yumurta ikizi olan olgularımızın her ikisinin koroner anjiografisinde ejeksiyon fraksiyonu %63, sol ön inen arterde (LAD), sirkümfleks (Cx) 2. obtus ve sağ koroner arterde darlık saptandı. Bunun sonucunda koroner bypass ameliyatına karar verildi. Eş zamanlı yapılan operasyonda pompadan çıkma döneminde miyokardiyal performansları aynı seyretti. Olguların her ikisinin de hemodinamileri, aktive edilmiş pıhtılaşma süresi (ACT), kan değerleri ve ilaç dozları benzer bulundu. Sonuç olarak tek yumurta ikizlerinde ameliyat bulgularının ve klinik seyrinin benzer olması, ameliyatın planlanması ve oluşabilecek kotnpükasy onların önleminin alınmasında faydalı olacaktır.
