Abstract

In this study, the outcome of 146 radical cystectomies for invasive bladder cancer were evaluated. Forty patients (27 %) had nontransitional cell carcinomas and 106 patients (% 73) had transitional cell carcinoma. Perioperative mortality was 2 %. Lymphatic involvement according to stages were 0 % in pT2,17.6 % in pT3 and 64 % in pT4 (p< 0.05). Survival data of 96 cases with transitional cell carcinoma were available in a mean follow-up of 34.4 months (range; 3-60 months). Patients with locally advanced disease (pT4 NO/+MO) were given 4 courses of adjuvant M-VEC (methotrexate, vinblastine, epirubicin and cis-platinum) chemotherapy. The 42-month disease specific survival rates ofpT2NOMO, pT3NOMO and pT4 NO/+MO patients were 89 %, 37.8 % and 9.8 %, respectively (p< 0.001). Survival rates for pT3-4NOMO and pT3-4N+MO cases were % 34.9 and % 6.7, respectively (p< 0.002). The findings suggest that the rate of lymph node involvement significantly rises with the increasing T stage, survival rates are low in locally advanced disease despite radical surgery and adjuvant chemotherapy, good results can be achieved with early radical cystectomy in muscle invasive tumors either confined to the bladder or with minimal perivesical invasion.

Özet

Bu çalışmada invazif mesane tümörü nedeniyle radikal sistektomi operasyonu geçiren 146 olgunun sonuçları değerlendirilmiştir. Kırk olguda (% 27) değişici epitel karsinomu dışında tümör ve 106 olguda (% 73) değişici epitel karsinomu mevcuttu. Perioperatif mortalite oranı % 2 olarak belirlendi. Evrelere göre lenfatik tutulum oranları pT2'de % 0, pT3'te % 17.6 ve pT4'te % 64 idi (p< 0.05). Ortalama 34.4 aylık (3-60 ay arasında) izlem süresinde 96 değişici epitel karsinomu olgusunun sağkalım verileri elde edilebildi. Lokal ilerlemiş hastalığı (pT4NO/+MO) bulunan olgulara 4 kür adjuvant M-VEC (metotreksat, vinblastin, epirubisin, sisplatin) kemoterapisi uygulandı. pT2NOMO, pT3NOMO ve pT4NO/+MO olguların 42 aylık hastalığa özgü sağkalım oranları, sırasıyla, % 89, % 37.8 ve % 9.8 olarak bulundu (p< 0.001). Sağkalım oranları pT3-4NOMO ve pT3-4N+MO olgularda, sırasıyla, % 34.9 ve % 6.7 olarak belirlendi (p< 0.002). Bu sonuçlar, T evresinin artmasına paralel olarak lenf nodu tutulumu oranlarının arttığını, radikal sistektomi ve adjuvant kemoterapiye rağmen lokal ilerlemiş hastalıkta yaşam oranlarının düşük olduğunu, mesanede sınırlı kas invazyonu gösteren veya minimal perivezikal yayılımı olan olgularda erken radikal sistektomiyle olumlu sonuçlar alınabileceğini düşündürmektedir.
