Abstract

Background: We evaluated Th17 group cytokine profile which is supposed to play an important role in inflammatory diseases, the Wnt pathway which is essential for bone formation and destruction, and parameters of osteoclastogenesis in psoriatic arthritis (PsA) patients. Materials and Methods: We included 48 PsA patients, 20 psoriasis patients and 19 healthy controls into the study. The clinical features of the patients were recorded down from hospital records. A dermatologist evaluated psoriasis area severity index (PASI) scores. ESR, CRP were determined. ELISA was used to determine Th17 group cytokines (IL-17, IL-22, IL-23), Wnt pathway inhibitor DKK-1, and osteoclastogenesis marker soluble RANKL levels. Results: In PsA group, IL-17 levels were significantly lower than in psoriasis and control groups (p values, 0.005 and <0.001). IL-22 levels in the control group were significantly higher than in PsA patients (p=0.001). PsA and psoriasis groups had significantly lower IL-23 levels than controls (p values, respectively, <0.001 and 0.014). The sRANKL level in the control group was significantly higher than in both PsA and psoriasis groups (p values <0.001). Serum DKK-1 levels in all groups were similar. The number of swollen joints in PsA patients correlated with IL-17 (r=053, p<0.001), DKK-1 (r=0.3, p=0.04) and PASI scores (r=0.28, p=0.05). Conclusions: We found lower levels of Th17 cytokines in PsA. A Th17 group cytokine, IL-17, correlated with the number of tender joints which is a reflection of the activity of PsA; and IL-22 correlated with sRANKL which is a marker of osteoclastogenesis.
