Abstract

Hyperimmunoglobulin E (hyper-IgE) syndrome (Job syndrome) is a rare immunodeficiency disease characterized by elevation of serum immunoglobulin E level, atopic-like chronic eczema, tendency for recurrent pyogenic infections pneumatoceles, reduced neutrophil chemotaxis and variable T cell function impairment. Forty-one years old male patient applied to our outpatient clinic with complaints of itching, redness, scaling, crusting and recurrent abscesses exacerbating especially during the winter months since his birth. Based on his clinical and laboratory findings a diagnosis of hyper-Ig E syndrome was made. He has previously been misdiagnosed as atopic dermatitis for a long time. Since it is rarely encountered and treatment-resistant we report this case.

Özet

Hiperimmünglobulin E (hiper IgE) sendromu (Job sendromu), serum immünglobulin E seviyesinde yükseklik, atopik dermatit benzeri kronik egzema, tekrarlayan piyojenik infeksiyonlara eğilim, pnömatosel, nötrofil kemotaksis bozukluğu ve değişken T hücre fonksiyon bozukluğu ile karakterize, nadir görülen bir immün yetmezlik hastalığıdır. Kırkbir yaşında erkek hasta, doğduğundan beri var olan, özellikle kış aylarında artan kaşıntı, kızarıklık, kabuklanma ve sık tekrarlayan apse şikayetleri nedeniyle polikliniğimize başvurdu. Hastaya mevcut klinik ve laboratuvar bulgularıyla hiper IgE sendromu tanısı kondu. Uzun bir süre farklı kliniklerde atopik dermatit tanısıyla takip edilmesi, tedavilere direnç göstermesi ve nadir görülmesi nedeniyle olgunun bildirimi uygun görülmüştür.
