Vincristine is a vinca alkaloid highly specific for cell cycle and frequently used in the chemotherapy protecol of many malignancies such as lymphoma and leukemia. The differential diagnosis of drug-induced neuropathy should be made absolutely. Vincristine neurotoxicity can be seen in four different ways: peripheral neuropathy, autonomic neuropathy, cranial nerve paralysis and encephalopathy. We present a case of a 13-year-old girl who developed partial intestinal obstruction after vincristine treatment with a diagnosis of Hodgkin's lymphoma. In our case; six days after the second vincristine dose in the second course of BEACOPP therapy, abdominal distension was developed with abdominal pain. Bowel sounds were normoactive and defense were available. Air liquid levels were detected in the direct abdominal graph on the foot. Ultrasonography of the abdomen revealed intense fluid, distension, and marked reduction in peristalsis in all intestinal anus. The patient was thought to be due to autonomic neurotoxic side effects of vincristine. Pridostigmine Bromide (Mestinon: 3 mg / kg /day, 2 doses, oral) and Pridoxin (B6: 150 mg / m2 / day, single dose, oral) were initiated in the patient. On the 2nd day of treatment, there was an increase in the intestinal sounds and a decrease in the abdominal distension. As a result, we wanted to emphasize that partial intestinal obstruction as a result of neuropathy caused by vincristine can be treated with pridostigmine and pyridoxine without any side effects.

Vinkristin; lenfoma ve lösemi gibi bir çok malignitenin kemoterapi protokolünde sıklıkla kullanılan hücre siklusuna oldukça spesifik bir vinka alkaloididir. İlaca bağlı nöropatinin ayırıcı tanısı mutlaka yapılmalıdır. Periferal nöropati, otonomik nöropati, kranial sinir paralizileri ve ensefalopati olmak üzere dört farklı şekilde vinkristin nörotoksisitesi görülebilmektedir. Hodgkin lenfoma tanısıyla vinkristin ted avisi sonrası parsiyel intestinal obstrüksiyon gelişen 13 yaşındaki bir kız olguyu sunuyoruz. Olgumuzda; BEACOPP tedavisinin ikinci küründe ikinci vinkristin dozundan 6 gün sonra karın ağrısı ile birlikte karın distansiyonu gelişti. Barsak sesleri normoaktif ve defansı mevcuttu. Ayakta direkt batın grafisinde hava sıvı seviyeleri saptandı. Batın ultrasonografisinde tüm barsak anslarında yoğun sıvı, distansiyon, peristaltizmde belirgin azalma ile hasta ileus tanısı aldı. Bu tablonun vinkristinin otonomik nörotoksik yan etkilerine bağlı olduğu düşünüldü. Hastaya Pridostigmin Bromür (Mestinon: 3 mg/kg/g, 2 dozda, oral) ve Pridoksin (B6 vitamini: 150 mg/m2/g, tek doz, oral) başlandı. Tedavinin 2. gününde bağırsak seslerinde artış, batın distansiyonunda azalma olduğu görüldü. Sonuç olarak, vinkristinin neden olduğu nöropati sonucunda gelişen parsiyel intestinal obstrüksiyonlarda; pridostigmin ve pridoksin ile tedavi edilebileceğini vurgulamak istedik.
