Abstract

Introduction: in retroperitoneal ftbrosis (RPF) ureters are involved by fibrosis and inflammation in the retroperitoneum and ureters are obstructed by the fibrotic mass. in 1905 Albarran and 1948 Ormond reported their experiences with this disease and therefore idiopatine form is also called Ormond's disease. Until 1958 management of RPF was surgical treatment which included ureterolysis and intraperitonealization. Recently various medical therapies have been tried for RPF, including steroids, tamoxifen and immunosuppressive agents. Here we present our experience with patients, who have been treated surgically or medically for idiopathic retroperitoneal fibrosis. Materials and Methods: Between June 1987 and February 2002 twenty-eight patients were treated with the diagnoses of idiopathic retroperitoneal fibrosis. Their data were analyzed retrospectively for renal function and progress of the disease. Sisteen patients were treated surgically and 12 patients were treated medically. The diagnoses were made by computerized tomography (CT) or intravenous pyelography (IVP) and erythrocyte sedimentation rate (ESR). Surgical therapy consists ureterolysis and intraperitonealization of both ureters. Medical managements of RPF for our 12 patients were steroid+tamoxifene or tamoxifene alone. Eight patients were treated with metil-prednisolone 32 mgr/d +tamoxifene 20 mgr/d, and 4 cases vvere treated only tamoxifene 20mgr/d. Results: Follow-up for surgically treated patients was 81.8 months (37-130) and for medically treated group was 13.8 months (6-52). in 13 patients out of 16 surgically treated groups, renal function could be preserved; only in 3 patients renal functions decreased during follow-up in 2-5 years post-operatively. These patients managed with steroid for 6 months, in two of them renal function stabilized, but one of these patients did not respond steroid treatment and hemodialysis was required. Initially medically treated patients followed up in every third months until the treatment ended. Medical treatment was stopped when fıbrotic mass was shrinking. The duration of medical treatment was 3-13 months (mean 5.6 months). in all medically treated patients renal function was preserved and the progression of disease was prevented at the beginning of therapy. Only in 2 patients 7 and 10 months after the treatment progression of the disease occurred. Steroid therapy was added for these patients, one of them responded to the therapy, but the other required surgical treatment. Conclusion: Although medically treated group had short follow-up period compared to surgically treated group, medical treatment option with low morbidity could also preserve renal function. Our opinion for treatment of RPF should begin with medical treatment, surgical therapy should be recommended only for not responders to medical therapy.
