Abstract

Introduction: In this study, we reviewed the results of percutaneous nephrolithotomy (PNL) in 108 renal units (RU). Materials and Methods: Between March 2005 and January 2006, 108 percutaneous nephrolithotomies (PNL) were done in 50 adult male, 45 adult female and 8 children with a mean age of 42.8 (5-75). 43 right, 65 left kidneys were involved. Age limit was taken 17 years in pediatric patients. Stone burden was calculated by multiplying lengths in millimeters of two edges of stones on direct urinary system graphy. All PNLs were done under general anesthesia with single dose antibiotic prophylaxis. Results: Average stone size was calculated as 500.36 mm2 (between 84 and 2000). The stone free rate was 73% (79/108). When residual fragments less than 4 mm were defined as "insignificant", success rate raised to 94 % (101/108). Separate analysis of 5 renal units with Staghorn calculi revealed 80% (4/5) complete stone clearance and 100% (5/5) success rate with insignificant residuals. The corresponding success rates in 18 lower pole calyx calculi were 89% (16/18) and 94%, respectively. A single session PNL was successful in 103 (95%) RUs, but repeat PNL/ESWL was necessary in the remaining 5 RU. Postoperative hemorrhage requiring transfusion was encountered in 7 (6 %) patients. Renal vascular angiography and selective embolization were performed in 1 patient because of long lasting bleeding from urinary tract. Mean operative time was calculated as 77.4 minutes (10-225 min). Double J stent as a diversion was used in 19 patients. Mean postoperative hospitalization day was calculated as 3.8 days (1-16 days). Supracostal access was done in 11 patients. In one patient, postoperative plevral effusion was seen and treated conservatively. Eleven patients were treated for urinary tract infection postoperatively. Conclusion: In view of these results, PNL appears as an effective and safe treatment modality for renal calculi.
