Abstract

Introduction: We investigate the diagnostic efficacy of digital rectal examination, prostate spccifıc antigen, transrectal ultrasound of prostate, prostate specific antigen density, age related PSA and free/total PSA in the prostate cancer. Materials and Methods: Between 1998 and 2002, we performed TRUS guided prostate biopsies to 233 patients (age 43-80, median 68) with suspicious digital rectal examination and/or PSA>4 ng/ml. We detected prostate cancer in 56 patients. The overall accuracy of abnormal TRUS fındings, PSAD 0,15 ng/ml/cc, age related PSA and F/T PSA (0,20 cut-off) were determined. Results: Cancer detection rate were; Only DRE: 0%, only PSA: 7.89%, DRE and PSA: 52%, DRE and PSA>10: 72%, PSA and TRUS: 62.5%, DRE and PSA and TRUS: 76%, Normal DRE and PSA: 4-10, PSAD: 0.15 cut-off: 14%. in age related PSA 70-79,11 biopsies avoided and 2 cancers missed in 81 patients. When the f/t PSA 0.20 cut-off point taken into consideration, 85% sensitivity was detected. Conclusion: Highest accuracy rate was provided with DRE -PSA>10 and DRE-PSA-TRUS combination. PSAD, age related PSA and f/t PSA rate did not provide more Information in prostate cancer detection.
