Abstract

Introduction: Erectile dysfunction (ED) is a common disorder in diabetic (DM) men. Although sildenafil is one of the most preferred treatment agents, significant numbers of these patients don't respond to this treatment. It is reported that some of these patients may benefit from vardenafil treatment. In this study, we evaluated the efficacy and safety of vardenafil treatment in diabetic patients with ED who had not responded to sildenafil mono-therapy before. Materials and Methods: Twenty-two men who had taken sildenafil properly and did not respond to this treatment were enrolled to the study. In addition to DM, 12 patients had hypertension, 11 had hypercholesterolemia, and 8 had atherosclerotic hearth disease. Vardenafil (20 mg) was given at least 4 doses to all the patients (maximum one dose per day before sexual activity). International Index of Erectile Function (IIEF)-erectile function (EF) domain score, percentage of positive responses to SEP2/ SEP3 questions and Global Assessment Question (GAQ) were used for assessment of erectile function before and after the treatment. Results: One patient discontinued the treatment due to severe headache and the remaining 21 patients completed the treatment protocol. Mean EF domain score was 9.3 at baseline and 10.2 at the end of the treatment (p>0.05). The positive response rates were found 19.1% for SEP 2 and SEP 3 and 23.8% for GAQ. Significant improvement in erectile function was observed in only 3 (14.2%) of the patients (p<0.05). Side effects were seen in 5 (22.7%) patients (headache in 4 patients and flushing in one patient) and one of them discontinued the therapy due to side effect. Conclusion: Vardenafil treatment did not found effective enough in diabetic patients with ED that sildenafil nonresponder.

SEP 2 ve SEP 3'e pozitif cevap oranları %19.1, GAQ'a pozitif cevap oranı %23.8 bulundu. Sadece 3 (%14.2) hastanın erektil fonksiyonlarında anlamlı bir düzelme gözlendi (p<0.05). 5 (%23.8) hastada ilaca bağlı yan etki gelişti (4 hastada baş ağrısı ve 1 hastada yüzde kızarıklık). Sildenafil tedavisine yanıt vermeyen erektil disfonksiyonlu şeker hastalığı olan erkeklerde vardenafil tedavisinin etkinliği yeterli bulunmamıştır.
