Abstract

Objective: In recent years, there has been a tendency to conservative management of renal trauma except for renal pedicle injuries. In this study, we evaluated conservative and surgical approaches in high-grade renal trauma and the effect of isolated and nonisolated renal injuries on the treatment approach and the outcomes. Materials and methods: We reviewed 40 patients (36 males; 4 females; mean age 21 years; range 2 to 51 years) with renal injuries. Sixteen patients were children. Injuries were blunt in 35 patients (87.5%), and penetrating in five patients (12.5%). The patients were evaluated by computed tomography and renal injuries were graded according to the AAST (American Association for the Surgery of Trauma) criteria. Results: Renal exploration was performed in 11 cases (27.5%) with blunt injuries, six of which required nephrectomy (54.6%). Thirty-three patients had grade 1-3 renal trauma. Of these, 30 patients (75%) recovered with conservative management, including five patients with grade 1-2 renal trauma who underwent emergency surgery for associated injuries without renal exploration. Four patients (10%) who underwent emergency exploration for associated injuries required renal exploration resulting in nephrectomy. Of these, three had grade 3, one had grade 4 injuries. One patient with isolated grade 4 renal injury required nephrectomy due to hemodynamic deterioration. The remaining five patients with isolated grade 4 injuries were treated conservatively. Of these, only one patient required nephrectomy due to early hemodynamic deterioration. During hospitalization, three patients required blood transfusion; extravasation of urine was seen in one patient, and hematuria and fever were seen in two patients. Conclusion: While a great majority of patients treated conservatively for isolated renal injuries recover without nephrectomy, associated abdominal injuries requiring emergency exploration often result in nephrectomy. Our findings favor conservative approach to renal injuries in patients who maintain hemodynamic stability.

Yaralanmaların 35’i künt (%87.5), beşi penetran (%12.5) yaralanma idi. Hastalar tedavi öncesinde bilgisayarlı tomografi ile değerlendirildi. Böbrek hasarı AAST (American Association for the Surgery of Trauma) ölçütlerine göre derecelendirildi. Bul gu lar: Renal eksplorasyon toplam 11 hastada (%27.5) yapıldı. So nuç: Konservatif yaklaşımla izlenen izole böbrek travmalı hastaların büyük çoğunluğu nefrektomi yapılmaksızın düzelirken, eşlik eden abdominal organ yaralanması nedeniyle yapılan acil eksplorasyonlar nefrektomi ile sonuçlanmaktadır. Bulgularımız, hemodinamisi stabil seyreden hastalarda böbreğe konservatif yaklaşımı desteklemektedir.
