Abstract

Aim: In this study, we aimed to determine the optimal infusion doses of dexmedetomidine which provides optımum hemodynamic and humoral stability under BIS guidance. Material and Methods: The study included 71 women (ASA physical status I-II) scheduled for total abdominal hysterectomy and bilateral salphingooferectomy. The patients were randomized into three groups. The patients received in random order 1μgr/kg dexmedetomidine load followed by infusions of either 0.2, 0.4 or 0.7 μgr/kgh-1 dexmedetomidine. The heart rate, blood pressure, respiratory rate, O2 saturation and BIS were monitored. Baseline measurements were acquiered and measurements were repeated at certain times. Anesthesia was induced with fentanyl 1μg/kg and propofol BIS quided until the lash reflex disappeared and maintaince with desflurane in 50% air and O2. desflurane was administered according to predetermined hemodynamic critaria. Aldrete recovery score was applied at the first and fifteenth minutes after extubation. Results: The two groups were similar in weight, age and duration of surgery. There was no difference between the means of BIS values and hemodynamic parameters before the surgery. After the induction, there were significant differences between the MAP values of three groups during 40th.,60th.,80th., 90th., 100th., 110th. minutes (P 0.005). There were significant reductions between control values and hemodynamic parameters in all three groups. All the neuroendocrin humoral responses to the surgery were not statisticaly significant. Postoperative adverse event rates were similar. The Aldrete recovery scores were similar between the groups. Conclusion: As a result dexmedetomidine infusion did not completely abolish the need for desflurane but diminished its requirement. The hemodinamic response to endotracheal intubation and skin incision was signifiantly blunted but was not able to attenued the stress response on surgery. The optimal dose of dexmedetomidine for maintaince of anaesthesia with desflurane appears to be in the range of 0.2-0.4 μgr/kgh-1.

Özet

Amaç: Bu çalışmada, dexmedetomidinin bispektral indeks (BIS) rehberliğinde optimum hemodinamik ve hümoral stabilitenin sağlanmasında en uygun infüzyon hızını araştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya total abdominal histerektomi-bilateral salpingo ooferektomi planlanan, ASA I-II statüsünde 71 kadın hasta alındı. Hastalar rastgele üç gruba ayrıldı. BIS, ortalama arter basıncı (OAB), kalp atım hızı (KAH), SpO2 (SAT) ve solunum sayısı (SS) kontrol değerleri kaydedildi. Sedasyon skorları Ramsay sedasyon skorlaması ile değerlendirildi. Stres hormon analizi için kan örnekleri alındı. Anestezi indüksiyonundan önce her üç gruba deksmetedomidin 1μgr/kg dozunda 10 dakika içinde uygulandı.
