Abstract

Between 1994 and 1998, 4513 adult tuberculosis patients were followed in our hospital. Miliary tuberculosis was diagnosed in 35 (0.8%) of them. We retrospectively evaluated medical records of these adult miliary tuberculosis patients for clinical, treatment related and prognostic features. Mean age of patients was 37.1 ±16.1 years. They were 17 male and 18 female patients. Two were relapse tuberculosis and others were new cases. History of contact with tuberculosis patient was 17.1%. Cough, weight loss, breath shortness were frequent complaints. Cachexia, fever, and rales were frequent findings on physical examination. Hepatomegaly and lymphadenopathy were seen 28.5% and, 25.7% respectively. Anemia and elevation of serum transaminases were usual laboratory findings. Typical miliary opacities were observed in 12 (34.2%) patients in chest radiography. Miscellaneous radiological lesions were seen in others additionally. Mycobacterium tuberculosis was shown bacteriologically in 20 (57.1%) and histopathologically in 8 (22.8%) patients. Tuberculosis had affected multiple organs in 20 (57.1%) patients. Miliary tuberculosis is a disease whose diagnosis is often difficult and may be still fatal even in adult and HIV negative population. In a person who is suspected to have miliary tuberculosis, diagnosis should be established as soon as possible and proper therapy should be administered immediately.

Özet

Hastanemizde 1994-1998 yılları arasında tedavi edilen 4513 erişkin tüberküloz hastasından 35'i miliyer tüberküloz tanısı almıştı.
