Abstract

The aim of our study was to demonstrate factors effecting the probability of to obtain a diagnosis with forceps biopsy in the patients examined with fiberoptic bronchoscopy for lung cancer. Seventy-seven bronchoscopic procedures were analysed retrospectively. Overall diagnostic yield of forceps biopsy was 57.1% (cytology not included). Ratio increased to 81.8% in central endobronchial lesions. In the results of multivariate logistic regression analysis, appearance and localisation of the lesion (odds ratios were 4.638 and 4.657 respectively) and appropriateness of biopsy pieces to diagnosis according to bronchoscopist idea (odds ratio= 2.932) were detected as a significant factors. Experience of bronchoscopist (duration of working as a specialist), existence of anatomical variations, endobronchial or submucosal lesions existence, biopsy procedure types were not relevant factors. Not absolute number of specimens but status of pieces (dimensions, appearance, crush etc.) was found more important. Experience of bronchoscopist in decision of adequateness of specimens was statistically significant factor in multivariate analysis. We think that cooperation with pathologist to ensure the appropriateness of biopsy specimens to histopathologic study and increasing the patient comfort to easily obtain adequate amount of specimen are important in obtaining maximum value from flexible fiberoptic bronchoscopy in the diagnosis of cancer.
