Abstract

Methotrexate is the most widely used second-line treatment in rheumatoid arthritis (RA) because of its efficacy. However, increasing number of cases of lymphoproliferative disorders have been reported in RA patients under methotrexate therapy. The risk factors for RA patients to develop lymphoma while under methotrexate therapy include severe disease, intense immunosupression, genetic predisposition and an increased frequency of latent infection with prooncogenic viruses such as Epstein-Barr virus. We aimed to describe a case of nodular sclerosing Hodgkin's disease developing in a 59 years old woman with seropositive RA treated with oral methotrexate 15 mg/week for 5 years. Computed tomography of the thorax revealed marked hiler lymphadenopathy and interstitial pattern. After discontinuation of methotrexate and adding adriamycin, bleomycin, vinblastine, dacarbazine chemotheraphy, a complete remission was achieved. The physicians caring for RA patients on methotrexate therapy should be vigilant with their patients about developing lymphoma.

Özet

Metotreksat, romatoid artrit (RA)'teki etkinliği nedeniyle ikinci basamak tedavide geniş kullanım alanına sahip bir ajandır. Ancak metotreksat kullanan RA'lı hastalarda artan sayıda lenfoproliferatif hastalıklar bildirilmektedir. Metotreksat tedavisi alan RA'lı hastalardaki lenfoma gelişimi risk faktörleri; ciddi hastalık, hassas immünsüpresyon, genetik predispozisyon ve "Epstein-Barr" virüs gibi proonkojenik virüslerin latent infeksiyonunun artan sıklığına bağlanabilir. Biz de beş yıldır oral olarak 15 mg/hafta da metotreksat kullanan 59 yaşındaki seropozitif RA'lı bayan hastada gelişen nodüler sklerozan tip Hodgkin lenfoma gelişimini sunmayı amaçladık. Olgunun toraks bilgisayarlı tomografisinde belirgin hiler lenfadenapati ve interstisyel patern mevcuttu. Metotreksat tedavisini kesip adriamisin, bleomisin, vinblastin, dakarbazin kemoterapisi uygulanan olguda tam remisyon elde edildi. Metotreksat tedavisi alan RA'lı hastaları takip eden hekimlerin hastalarında olası bir lenfoma gelişimi yönünden uyanık olmaları gerekmektedir.
