Abstract

In this study, three cases, which developed acute respiratory distress syndrome (ARDS) secondary to pneumonia, followed in respiratory intensive care unit (RICU) were evaluated retrospectively. Their ages ranged between 13 and 47 years and two of them were female. First-RICU-day Acute Physiologic Assessment and Chronic Health Evaluation II (APACHE II) score was 21.7 ± 7.6 (mean ± SD); APACHE III score was 74 ± 21.2 (mean ± SD). The mean length of RICU stay was 5.3 ± 3.2 days. All of the cases were mechanically ventilated during RICU. Protective lung ventilation strategy with low tidal volume and PEEP had been used for the first case. Prone ventilation and inverse ratio ventilation had been used for the second case whereas conventional mechanical ventilation and PEEP had been used for the third case. Two cases died at the third and fourth days following admission to RICU. In conclusion, underlying diseases, early diagnosis, triggering factors and treatment approaches determine the prognosis of ARDS that has high mortality. We thought that outcome would be favourable in both diagnosis and treatment process with the presentation of clinical data and experiences associated with ARDS.

Özet

Bu yazıda; pnömoni nedeniyle akut respiratuar distres sendromu (ARDS) gelişen, solunumsal yoğun bakım ünitesi (SYBÜ)’nde izlediğimiz, yaşları 13 ile 47 arasında değişen, ikisi kadın üç olgu retrospektif olarak değerlendirilmiştir. SYBÜ’deki ilk 24 saate ait “Acute Physiologic Assessment and Chronic Health Evaluation II (APACHE II)” skoru 21.7 ± 7.6 (ortalama ± SS), APACHE III skoru ise 74 ± 21.2 (ortalama ± SS) idi.
