Abstract

In this case report our purpose is to emphasize one-lung ventilation with double-lumen tube in pediatric patients as a rare technique. However one-lung ventilation is used frequently in older cases underwent thoracic surgery. A six yearold child was hospitalized with the diagnosis of right-sided pneumothorax and subcutaneus emphysema secondary to traumatic bronchial rupture. The case was hypotensive, tachycardic and tachypneic. Peripheric oxygen saturation was 90% and arterial blood gas analysis with nasal oxygen was represented as pH: 7.101, PaO2: 61.4 mmHg, PaCO2: 72.2 mmHg, HCO3: 21.9 mmol/L, BE: -7.7 mmol/L. Right thoracotomy and anastomosis of right mainstem bronchus were planned and the case was taken into the operating room. After induction of general anesthesia, diagnostic rigid bronchoscopy was performed and disruption of right main bronchus from intermedier bronchus was observed. Then the case was intubated with 26 Fr left Robertshaw double-lumen tube and one-lung ventilation was started. Two-lung ventilation was applied after right thoracotomy, upper lobectomy and anastomosis of right main bronchus. We didn't encounter any complication during and following operation. He was extubated and transferred to intensive care unit.

Özet

Bu olgu sunumunda amacımız; göğüs cerrahisinde erişkin olgularda sık kullanılmasına rağmen, çocuklarda nadir kullanılan bir yöntem olan çift lümenli tüple tek akciğer ventilasyonunun vurgulanmasıdır. Cerrahi yoğun bakım ünitesinde travmatik bronş rüptürü tanısıyla izlenen altı yaşındaki olgunun sağda pnömotoraks ve deri altı amfizemi mevcuttu. Olgu hipotansif, taşikardik ve takipneikti. Nazal O 2 alırken periferik oksijen satürasyonu %90 ve arteryel kan gazı analizinde; pH: 7.101, PaO 2 : 61.4 mmHg, PaCO 2 : 72.2 mmHg, HCO 3 : 21.9 mmol/L, BE: -7.7 mmol/L idi.
