Abstract

Fortysix (1.06%) superior sulcus tumor cases of 4340 cases with primary bronchial carcinom diagnosed in our clinic between 1994-1998 were evaluated retrospectively for age, symptoms and findings, anatomic localisation, histology, diag-nostic methods, staging. All of the cases were male and mean age was 52 (21 -70) years. 89.1 % of the cases had shoulder pain and 41.3% had arm pain. Horner syndrome was present in 21.7% of cases and Pancoast syndrome in 6.5% of cases. The duration between the beginning of symptoms and the diagnosis was 5 months. Radiologie localisation was posterior in 69.6% of cases, anterior in 21.7%. Chest wall invasion was present in 71.7% of cases, costa destruction in 54.3% of cases, subclavian vessel involvement in 36.9% of cases. The diagnosis was reached via transthoracic fine needle aspiration (TTFNA) in 73.9% of cases, bronchoscopic methods in 19.5%, transthoracic through-cut biopsy in 13%. The diagnostic value was found 97.1% for TTFNA, 45% for bronchoscopic methods, 100% for transthoracic through-cut biopsy. Squamous cell carcinoma was detected in 43.4% of cases, adenocarcinoma in 11% of cases, large cell carcinoma in 4.3% of cases and small cell carcinoma in 4.3% of cases. 30.4% of cases were in stage MB, 6.5% in stage MIA, 32.6% in stage IHB and 28.2% were in stage IV.Because of the special localisation of these tumors, the cases were examined in orthopedics arid physical therapy clinics for shoulder and arm pains for months. Thorax computerized tomography is used for detailed examination of local distribution of these tumors and diagnosis reached via transthoracic approaches.
