Abstract

A rare case öf endobronchial tuberculosis mimicking lung cancer on computerized tomography (CT) of thorax and bronchoscopic examination is reported. A 45-year-old female applied with a symptom of chronic cough, a mass lesion on right hilum reported on CT of thorax. Fiberoptic bronchoscopy revealed an endobronchial mass lesion on the orifice of right main bronchus. Pathological biopsy reported granulomatous inflammation and acid-fâst bacilli smear was found positive in bronchial lavage fluid. Anti-tuberculous treatment was given for endobronchial tuberculosis. Because lung tuberculosis is frequently seen in our country, endobronchial tuberculosis must be suspected especially in cases with chronic cough. Endobronchial tuberculosis can mimic lung cancer with clinical and radiological findings, so suspected cases should be examined for tuberculosis on bronchoscopic materials.

Özet

Nadir rastlanan bir klinik antite olması nedeniyle toraks bilgisayarlı tomografisi (BT)'nde ve bronkoskopik muayenede maligniteyi taklit eden bir endobronşiyal tüberküloz olgusu sunulmuştur. Kırkbeş yaşındaki kadın olgu, kronik öksürük şikayetiyle başvurmuş, çekilen toraks BT'de sağ hilusta kitle lezyonu tespit edilmiştir. Yapılan fiberoptik bronkoskopide sağ ana bronş girişinde endobronşiyal kitle lezyonu izlenen olgunun biyopsi sonucu granülomatöz inflamasyon olarak raporlanmış ve bronş lavaj sıvısında, aside dirençli basil direkt bakıda pozitif değerlendirilmiştir. Hastaya endobronşiyal tüberküloz tanısıyla antitüberküloz tedavi başlanmıştır. Ülkemizde akciğer tüberkülozu yaygın olduğundan, özellikle kronik öksürük şikayetiyle başvuran hastalarda endobronşiyal tüberkülozdan şüphe edilmelidir. Endobronşiyal tüberküloz klinik ve radyolojik olarak akciğer kanserini taklit edebileceğinden şüphe edilen olgularda bronkoskopik materyallerde tüberküloza yönelik tetkikler yapılması uygun olabilir.
