Abstract

Aim: Thoracic injury is responsible for the 25% of deaths following travma. Sternal fractures (SF) are seen like 3-8% after blunt thoracic trauma. The aim of our study is to discusse the diagnosis and treatment of SF by the data obtained from these patients. Material and methods: In our study we retrospectively analyzed 17 patients with SF; etiology, symptoms, clinical findings, coexisting poundings, myocardial injury findings, imaging methods and treatment modalities were inspected. Results: Most common cause for SF in our cases were falling down from a high place (58.8%, n=10). SF was most commonly located in the middle of corpus sterni (n=10). In two patients both middle of corpus sterni and manibrium were fractured. All the fractures were transversely located. Coexisting poundings were rib fractures in 2, carnial trauma in 1 and extremity fracture in 1 patient. Although pain and sensitivity were seen in all patients, crackle was seen in only 3 patients. Cardiac enzyme profiles were elavated in 3 patients which decreased in the follow up. There was an organ pounding outside the thoracic cavity in 1 patient. Sternal fixation was applied to 2 patients. One had deplaced sternum and the other had a stabilization defect in the anterior chest wall due to multiple rib fractures. The case with multiple rib fractures died after surgery due to renal failure. Treatment of SF, is same as in rib fracture and consists of pain control and pulmonary hygiene. Severe deplaced sternum and SF with flail chest are rare cases and require additional treatment. Most of our cases were followed up without an invasive intervention (88.2%, n=15). Cardiac enzymes were elevated in 3 patients but they were all hemodynamically stable. Conclusion: For the hemodynamically unstable patients, with changes in ECG, and elavated cardiac enymes, myocardial pounding should be kept in mind.

Özet

Amaç: Travma sonrası ölümlerin yaklaşık %25?i toraks yaralanmalarına bağlıdır. Sternum fraktürleri (SF), künt toraks travmaları sonrası %3-8 oranında görülür. Çalışmamızın amacı olguların ışığında SF?nin tanı ve tedavisini gözden geçirmekti. Gereç ve yöntem: Bu çalışmada 17 SF?li hastanın bulguları retrospektif olarak incelendi; etiyoloji, semptomlar ve bulgular, eşlik eden yaralanma, miyokard hasar bulguları, görüntüleme yöntemleri, tedavi şekilleri araştırıldı. Bulgular: Olgularımızda en sık rastlanılan neden yüksekten düşmeydi (%58.8, n=10). Fraktür genellikle korpus sternumun orta kesimindeydi (n=10).
