Abstract

Rapid recovery of right homonymous hemianopsia (HH) with medical treatment in a case of left occipital cortex ischemia is described. A 48-year-old white male with a history of cardiac bypass surgery, hypercholesteremia, and heavy smoking had calcified atheroma plaques located in the bilateral carotid arteries. Left occipital cortex ischemia developed after an embolic attack to the left posterior cerebral artery from the internal carotid artery. Intravenous high-dose methylprednisolone associated with hyperbaric oxygen, antiaggregant, anticoagulant, antioxidant, and ocular hypotensive therapy was initiated within 24 hours of the incident. Rapid recovery of right HH was observed within a week of treatment. This case illustrates the beneficial effect of high dose intravenous steroid associated with hyperbaric oxygen, antiaggregant, anticoagulant, antioxidant, and ocular hypotensive therapy for HH.

Özet

Oksipital korteks iskemisi nedeniyle gelişen homonim hemianopsi (HH)‘nin medikal tedaviye kısa sürede verdiği hızlı cevap olgumuzda sunulmaktadır. Geçirilmiş kardiak bypass cerrahisi, hiperkolesteremia ve yoğun sigara içme öyküsü bulunan 48 yaşında erkek hastada bilateral internal karotid arterlerde yerleşim gösteren kalsifiye atherom plaklarından sol posterior serebral artere emboli sonucu sol oksipital korteks iskemisi gelişmiştir. Olgumuza ilk 24 saat içinde uygulanan yüksek doz intravenöz metilprednizolon, hiperbarik oksijen, antiagregan, antikoagulan, antioksidan ve oküler hipotansif tedavi ile birinci hafta sonunda HH’de hızlı bir düzelme izlenmiştir. Ani gelişen HH’lerde ilk 24 saat içinde hiperbarik oksijen, antiagregan, antikoagulan, antioksidan ve oküler hipotansif ajanlarla birlikte uygulanan yüksek doz intravenöz streoid tedavisinin etkinliği olgumuzda gösterilmiştir.
