Abstract

Objective: In term fetuses, to compare the measurement of the cervix by transvaginal ultrasound with Bishop score and parity to predict successful labor induction. Methods: In this prospective study, ninety-seven pregnant women undergoing labor induction between 37-42 weeks of gestation were enrolled in this study. The Bishop score was assessed by digital examination whereas the cervical length was measured by transvaginal sonography. It was investigated the impact of cervical length, Bishop score, parity, dilatation, effacement, age and induction agent on induction to delivery interval and the risk of cesarean section. Pearson correlation, multiple linear and logistic regression model and ROC curve were used as statistical methods. Furthermore Mann Whitney U test for continuous and chi-square test for categoric variables were used. Results: Cesarean delivery occured in 18,6 % of the patient. There were a correlation between induction to delivery interval and cervical length, cervical dilatation, effacement and Bishop score (r: 0,448 P<0,0001, r:-0,382 P<0,0001, r:- 0,357 P:0,02, r:-0,261 P:0,01, respectively), whereas it was not found a correlation with parity, age and induction agent. In multiple linear regression model, the cervical length is the only independent factor predicting induction to delivery interval (t:4,247, 95 % CI: 0,146-0,404 P< 0,0001). Alo, in logistic regression model for cesarean probability, the cervical length is the only independent factor (OR:1,114 95%CI: 1,024-1,211 P <0,05). Receiver-operating characteristics (ROC) curves for predicting cesarean delivery demonstrated that the best cut off value was 18 mm for cervical length and cervical length was better for predicting of cesarean delivery than Bishop score (area under curve = 0,709 versus 0,390). The patients with cervical length <18 mm were associated with lower cesarean rate comparing patients with cervical length >18 mm (%0 versus %22, sensitivity and specificity %100 and % 23 respectively). Conclusion: In patients undergoing induction of labor, only the cervical length provide significant independent prediction of the likelihood of caserean section and induction to delivery interval. It doesn’t seem parity and Bishop score add significant independent information.
