Abstract

Aim: The goal of this study was to investigate the clinical value of the Tei index, which combines parameters of both systolic and diastolic ventricular function, in the assessment of left ventricular thrombosis risk after acute myocardial infarction (AMI). Materials and Methods: Echocardiographic examinations were performed in 78 consecutive patients with first anterior AMI within 24 hours after arrival to the coronary care unit. The Tei index, defined as the sum of isovolumetric contraction time and isovolumetric relaxation time divided by ejection time, was calculated. In order to analysis of left ventricular (LV) thrombus formation, 2-dimensional echocardiographic examination was repeated on days 3, 7, 15 and 30. Results: LV thrombus was found in 24 (30.8%) of 78 patients (Group 1) and was not found in 54 (69.2%) patients (Group 2). The index was significantly higher in Group 1 than in Group 2 (0.79±0.22 vs 0.68±0.19, p=0.037). When the index $\geq$0.75 was used as cutpoint, LV thrombus formation could be predicted with a sensitivity of 67%, a specificity of 65%, a positive predictive value of 46% and the negative predictive value of 81%. Conclusion: The Tei index can be used for assessing LV thrombus formation risk after AMI. The patients with high index seem to be at higher risk for thrombus formation.

Özet

Amaç: Çalışmamızın amacı, sistolik ve diyastolik ventriküler fonksiyonun ortak parametresi olan Tei indeksinin, akut miyokard infarktüsü (AMİ) sonrası sol ventrikül trombüsü gelişimi riskini incelemedeki değerini araştırmaktır. Gereç ve Yöntem: İlk kez anteriyor AMİ tanısı alan 78 ardışık hastada, koroner yoğun bakıma alındıktan sonraki ilk 24 saat içinde ekokardiyografik inceleme yapıldı. İzovolümetrik kasılma ve gevşeme zamanları toplamının ejeksiyon zamanına bölünmesi ile elde edilen Tei indeksi hesaplandı. Sol ventrikül (SV) trombüsü gelişiminin tespiti amacıyla 3, 7, 15 ve 30. günlerde 2 boyutlu ekokardiyografi tekrarlandı. Bulgular: Yetmişsekiz hastanın 24 tanesinde (%30.8) SV trombüsü tespit edildi (Grup 1). Kalan 54 hastada (%69.2) ise trombüs tespit edilmedi (Grup 2). İndeks Grup 1'de Grup 2'ye oranla anlamlı düzeyde daha yüksek bulundu (0.79±0.22 ve 0.68±0.19, p=0.037). İndeksin $\geq$ 0.75 olması kesim noktası olarak alındığında, SV trombüsü gelişimi %67 duyarlılık, %65 özgüllük, %46 pozitif tahmin değeri ve %81 negatif tahmin değeri ile öngörülebildi. Sonuç: Tei indeksi AMİ sonrasında SV trombüsü gelişimi riskinin değerlendirilmesinde kullanılabilir. İndeksi yüksek olan hastalar, trombüs gelişimi açısından yüksek riskli görünmektedirler.
