Abstract

Aim: It is well-known that the relation between insulin resistance and hypertension, the insulin resistance is being accepted as a risk factor for cardiovascular events. Some studies point out that there is a relationship between the angiotensin II and insulin resistance and that angiotensin converting enzyme inhibitors and angiotensin II receptor blockers are decreasing the insulin resistance. The aim of this study is to establish the relationship between valsartan, an angiotensin II receptor blocker, and insulin resistance in patients with essential hypertension. After 8 weeks therapy we examined the effect of valsartan on insulin resistance with 36 patients. Material and Methods: The insulin resistance has been studied with CIGMA (Continuous Intravenous of Glucose with Model Assesment) test on the patients all. Pre-treatment insulin resistance was detected 20 of 36 patients. All patients were received valsartan 80mg/day single dose. After the 8 weeks treatment, the patients were, physically examined again and CIGMA test repeated. After the treatment of valsartan, significant decrease of systolic and diastolic blood pressure was figure out. Before and after the treatment on the value of cholesterol triglyceride and glucose values the insignificant changes were observed. Results: Before the treatment, when we evaluated the 20 patients, whose insulin resistance were found out in advance, the insulin resistance was about average R: 2.70 in this group, after the tratment it was R: 2.05. The difference between (2.70-2.05) 0.65 (24%) was not statistically significant (P>0.05). Conclusion: According to the result of this study: it has been figured out that valsartan, an angiotensin II receptor blocker, has enough antihypertensive effect and no effect on glucose and lipid metabolism. Although valsartan's effect on insulin resistance was not statistically significant, in our study, we showed that there was little effect. Additional large experimental studies must be performed for exactly establish the relationship between insulin resistance and valsartan.
