Abstract

Aim: Oral apthous ulcers and skin lesions are the primary manifestations of Behcet’s disease (BD) activity. To date, there is no study investigating cardiovascular prognostic implications of these lesions. Objective: We hypothesized that right ventricular (RV) diastolic functions might have been more severely impaired within the active disease period with oral and skin lesions. Matherial and Methods: Forty-one patients with BD and age and sex matched 34 healthy control subjects were included. Subjects with at least 30 days of lesion free period were regarded within inactive disease period, and subjects with any oral and/or skin lesion was regarded within active disease period. Each subject underwent right ventricular echocardiography examination during both the inactive and the active disease period. Results: High sensitive C-reactive protein values were significantly higher in the BD patients both in inactive, and in active disease period compared to the controls (3.62±4.03 vs. 1.59±0.98; P= 0.02; and 3.62±4.03 vs. 18.82±15.85, P<0.001). Conventional and tissue Doppler echocardiographic examination revealed that BD patients within the inactive period had impaired right ventricular diastolic functions compared to the controls, and the right ventricular diastolic function was more prominently impaired during the active disease period compared to that of the inactive disease period in BD patients. RV diastolic parameters in the inactive and in the active disease period correlated to hsCRP. Conclusion: Right ventricular diastolic function is more prominently impaired in BD patients within the active disease period. BD patients are possibly more vulnerable to cardiovascular manifestations when they are in active disease period.
