Abstract

Aim: In this study, we aimed to evalute the effect of rheumatoid arthritis (RA) on atrial electromechanical delay, which is a noninvasive determinant of atrial fibrillation (AF) development risk. Material and Medhod: 47 patients with RA and 35 healthy individuals were enrolled in our study. The time interval between the onset of P wave on electrocardiography (ECG) and late diastolic wave on tissue Doppler imaging (TDI) was measured. Intraatrial and interatrial electromechanical delays were defined. Results: Interatrial and intraatrial electromechanical delays were found to be longer in patients with RA than in the controls (40.5±12.9 ms vs 21.4±7.8 ms; p<0.0001, 16.7±9.2 ms vs 6.9±4.1 ms; p<0.0001). A positive correlation was found between interatrial electromechanical delay and P wave dispersion (PWD), inflammatory markers such as hsCRP, erythrocyte sedimentation rate (ESR), leucocyte count (r=0.40, p<0.001; r=0.44, p=0.002; r=0.43, p= 0.003; r=0.34, p= 0.01) and the duration of the disease (r=0.53, p<0.0001). Conclusion: Atrial electromechanical delay is longer in patients with RA than in healthy individuals and this finding may point out the increased risk of atrial arrhythmia development by causing atrial structural changes because of the long-lasting inflammation in patients with RA.

Özet

Amaç: Çalışmamızda atriyal fibrilasyon (AF) gelişme riskinin noninvazif belirleyicisi olan atriyal elektromekanik gecikme üzerine Romatoid Artrit’in (RA) etkisini değerlendirmeyi amaçladık.
