Abstract

AIM: Diagnostic testing to evaluate the presence of viable but dysfunctional (hibernating) myocardium has become an important component of the clinical assessment of patients with coronary artery disease. Impaired left ventricular (LV) function in such patients is not always an irreversible process. Because LV function may improve considerably after myocardial revascularization. Thus identification of viable myocardium is important. METHODS: In this study, low (5-10 mg/kg per minute) and high (20-40 mg/kg per minute) dose dobutamine stress echocardiography (DSE) was performed in 27 patients with coronary artery disease and left ventricular dysfunction before coronary bypass surgery (CABG). Wall motion was scored from 1 to 4 (normal, hypokinesia, akinesia, dyskinesia) and functional improvement was defined as a score decrease 1 or more grade. Functional improvement during low dose and worsening at high dose dobutamine infusion was defined biphasic response. 72 akinetic, 59 hypokinetic, 2 dyskinetic, total 133 segments were evaluated. Functional improvement in dysfunctional segments, at third months after CABG was compared with preoperative DSE. RESULTS: In the our study low dose DSE in predicting functional improvement had a 72 % sensitivity in akinetic segments. 93 % in hypokinetic segments, 83 % in all dysfunctional segments. DSE had a 93% specificity in akinetic segments, 69 % in hypokinetic segments, 85 % in all dysfunctional segments. Specificity and sensitivity increased with biphasic response during maximal dose dobutamine infusion. Sensitivity was 90 % in akinetic segments, 89 % in hypokinetic segments, 89 % in all dysfunctional segments. Specifity was 100 % in akinetic, 78 % in hypokinetic and 87 % in all dysfunctional segments. In our study, CABG had been more beneficial in patients with severe left ventricular dysfunction (EF < 45 %). CONCLUSION: DSE can be used to predict improvement in left ventricular systolic function after CABG and biphasic response during dobutamine infusion is highly predictive of improvement after revascularization.
