Abstract

Aim: This study is designed to assess renal functions of patients experiencing low cardiac output syndrome and so taking positive inotropic drugs after coronary artery bypass surgery. Materials and Methods: Twenty six patients who had undergone coronary artery surgery by cardiopulmonary bypass and had experienced low cardiac output syndrome between September 2001 and February 2002 in our clinic were included in this study. Patients' demographical properties were simultaneously assessed with their renal functions. These parameters were noted preoperative, postoperatively while taking positive inotropic drugs at 24th and 36th hours and also 24th-36th hours' results after this regimen has stopped. Results: Mean age of patients included in this study was between 62.04±8.3. There were 15 male patients (%57.7), 11 female patients (%42.3). Intra-aortic balloon pump were used among 15 of patients additional to positive inotropic support regimen. When we compared the pre and postoperative results after stopping positive inotropic support we found decreased urinary protein (p<0.001) and fractional sodium excretion (p<0.001), but also increased creatinin clerence (p<0.001). Conclusion: Renal functions were seemed to be negatively effected after using positive inotropic drugs in patient having low cardiac syndrome. But this effect could not to be distinguished in between whether it depends to direct influence of inotropic agents or refers to underlying low cardiac output syndrome, so large scale study is needed to distinguish this difference.

Özet

Amaç: Koroner arter cerrahisi sonrası gelişen düşük kardiyak debi sendromu nedeniyle pozitif inotropik ajan kullanılan hastalarda, bu ajanların renal fonksiyonlar üzerindeki etkilerini değerlendirmektir. Gereç ve Yöntem: Kliniğimizde Eylül 2001-Şubat 2002 tarihleri arasında kardiyopulmoner bypass ile koroner arter cerrahisi yapılmış ve postoperatif dönemde düşük kardiyak debi sendromu nedeniyle pozitif inotropik ajan kullanılmış 26 hasta incelenmiştir. Hastaların demografik özellikleriyle beraber, preoperatif, postoperatif inotrop alırken 24.-36. saatlerdeki ve inotrop kesildikten sonraki 12.-24. saatlerdeki renal fonksiyonları biyokimya testleri ile değerlendirilmiştir. Bulgular: Çalışmaya alınan hastaların yaş ortalaması 62.04±8.30 olup 15'i erkek (%57.7), 11'i kadın (%42.3) idi.
