Abstract

AIM: Although valsalva manoeuvre (VM) is used to separate patients with pseudonormalization pattern of transmitral flow from normal pattern. Cases in echocardiography, a sound correlation could not be shown between blood pressure response to VM echocardiographic findings of cardiomyopathy patients. In this study was planned to investigate whether blood pressure response to VM provides any contribution to differentiate cardiomyopathy patients demonstrating pseudonormalization pattern in restrictive phase and those demonstrating relaxation abnormality. MATERIAL AND METHOD: Thirty-five consecutive patients (three woman and 32 man) with New York Heart Association (NYHA) class II to IV heart failure and ischemia cardiomyopathy followed in our hospital were included to the study. Ischemic CMP was diagnosed angiographically proven coronary artery disease history, a left ventricular end-diastolic dimension of>60 mm and an ejection fraction < 40%, Absence of a reduced systolic blood pressure in phase 2 of VM was evaluated as abnormal blood pressure response to MV. RESULT: While 13 patients had a normal VM response (group A), 22 had an abnormal response (group B). Doppler transmitral filling patterns put forward statistically significant differences. Higher transmitral E (p<0.001), lower transmitral A values (p<0.05) and higher E/A ratio (pseudonormalization pattern) were found in group B (p<0.001) concluding tendency to pseudonormalization pattern compared with group A. It was demonstrated that all of the group A patients had a transmitral E/A ratio < 1.0, however 18 (82%) group B patients had a ratio >1.0 (pseudonormalization or restrictive pattern) and four (18%) of them had a ratio <1.0 (p<0.01). CONCLUSION: Abnormal blood pressure response to VM is associated with moderate or severely disturbed. LV diastolic function in cardiomyopathy patients. Thus, it is suggested that this method could help us to predict the degree of severity cardiomyopathy.

BULGULAR: İskemik KMP 'li 13 hastanın VM'na kan basıncı cevabı normal iken (grup A), 22 hastanın anormal cevabı (grup B) vardı. Grup B hastalarda, Grup A hastalara göre daha yüksek E (p 1 iken (p<0,01, psödonormalize veya restiriktif örnek), sadece 4 hastada (%18) E/A<1 idi. SONUÇ: VM'na anormal kan basıncı yanıtı iskemik KMP 'li hastaların sol ventrikül diyastolik fonksiyonlarında orta ve ileri düzeyde kötüleşmeyle birliktelik göstermektedir. Böylece iskemik KMP 'li hastaların hangi evrede oldukları ayrımını yapmamıza yardımcı olabilir.
