Abstract

AIM: The aim of this study was to investigate the restenosis rate after percutaneous translüminal coronary angioplasty (PTCA) or intracoronary stent application in the diabetic patients. MATERIAL AND METHODS: The study group consisted of 64 diabetic patients (41 men {64. l%} and 23 women {35.9%}, mean age 57+9 years, range 35-76 years). A stent was placed on 37 native coronary artery lesions of 34 patients, and PTC A was performed for 40 native coronary artery lesions of 30 patients. Coronary angiography was repeated in all patients after a follow-up period of six months. Student-t test, chi-square test and Scheffe-F test were used for the statistical analyses, and correlation analyses were performed by using Pearson correlation coefficient. RESULTS: The primary success of the procedure was similar in both groups. After six months, control angiograms showed that restenosis occured in 19 (47.5%) of 40 lesions in the PTCA group and in 14 (37.8%) of 37 lesions in the stent group. This difference was not statistically significant. In A and C type lesions, the occurrence of restenosis was not affected differently by PTCA and stenting procedures; however in B type lesions, less restenosis was observed in the stent group compared to the PTCA group (p=0.01). The increase of the applied pressure resulted in more restenosis in the PTCA group (p=0.03), on the contrary it decreased the rate of restenosis in the stent group (p=0.01). In the PTCA group, no correlation could be found between restenosis and balloon diameter and length. On the other hand, it has been determined that small stent diameter and increased stent length augmented the rate of restenosis in the stent group (p=0.001 and p=0.02 respectively). In the PTCA group, any effect of the reference luminal diameter, the basal minimal luminal diameter and the post-procedure minimal luminal diameter on restenosis could not be found. However, all of these three parameters had an impact on restenosis in the stent group: Lower values of the reference luminal diameter and basal and post-procedure minimal luminal diameters caused a significant increase in the rate of restenosis (p=0.03, p=0.02 and p=0.02, respectively). CONCLUSIONS: A slight, but non-significant decrease in the rate of restenosis was observed with intracoronary stent application compared to PTCA in patients with diabetes mellitus. This difference was statistically significant in the subgroup with B-type lesions. To decrease the probability of restenosis, the shortest stent suitable to the lesion and the widest stent fitting with the reference vessel diameter should be chosen and an optimally high inflation pressure should be applied.

Özet

AMAÇ: Diyabetik hastalarda perkütan translüminal koroner anjioplasti (PTKA) veya İntrakoroner stent uygulamasının restenoz üzerine olan etkilerini, karşılaştırmalı olarak araştırmak amacı ile 41 'i erkek (%64.1), 23 'ü kadın (%35.9) toplam 64 diyabetik hasta incelendi.GEREÇ VE YÖNTEM: Hastaların yaş ortalaması 57±9, yaş aralığı 35-76 yıl idi. Çalışma grubundaki 34 hastanın. 37 nativ koroner damar lezyonuna intrakoroner stent, 30 hastanın 40 nativ koroner damar lezyonuna ise PTKA uygulandı. Girişimci tedavi sonrası 6 aylık takip döneminin sonunda yeniden koroner anjiografik tetkik yapıldı. İstatistiksel analizlerde stUdent-t, chi-square, Scheffe F testleri kullanıldı, korrelasyon analizleri Pearson korrelasyon katsayısı kullanılarak yapıldı. BULGULAR: Her iki grupta primer işlem başarısı eşit bulundu.
