Abstract

Implantable cardioverter-defibrillator is the dominant therapeutic modality for patients with life threatening ventricular arrhythmia's. They have high-energy shocks, antitachycardia and as well as antibradycardia pacing properties. Complications including lead extraction have been described by many physicians since newly developed transvenous electrode systems were used. We may extrapolate the pacemaker lead removal to ICD lead extraction, however ICD leads exhibits specificity's related to their particular design and large diameter. We report the case with ventricular tachycardia who requires lead removal due to the insulation defect. The patient underwent total removal of the defibrillator system and reimplantation was performed on the right pectoral region due to the left subclavian vein stenosis documented with prior subclavian vein catheterization. We did not observed any problem with new device on the right side and defibrillation threshold was good enough to provide a safety margin of at least 10 Joule.

Özet

Takılabilir kardiyoverter defibrilatörler (ICD) günümüzde hayatı tehdit eden ventriküler aritmilerin tedavisinde önemli bir yöntem olarak yerini almıştır. İCD' ler yüksek şok enerjisine sahip, antitaşikardik ve antibradikardik uyarı özellikleri bulunan cihazlardır. Gelişen teknolojiyle kullanıma giren yeni transvenöz elektrod sistemleri ekstraksiyona kadar giden yeni komplikasyonları gündeme getirmiştir. ICD elektrotlarının ekstraksiyonu "pacemaker" elektrotu ekstraksiyonuna benzerlik göstermesine rağmen özel yapısı ve geniş çapından dolayı birtakım farklılıklar gösterir. Elektrottaki insülasyon bozukluğundan dolayı sistemi değiştirilen ventriküler taşikardili olgumuzda eski elektrota bağlı olarak gelişen sol subklavian ven darlığı sonucunda yeni sistem sağ pektoral bölgeye takıldı. Sağ pektoral adele içine yerleştirilen sistemle yeterli defibrilasyon eşiği sağlanabildi ve 10 J'lük emniyet sınırı bırakıldı.
