Abstract

AIM: Previous studies have shown that QT dispersion increases during acute myocardial infarction (AMI). A relation between infarct size and QT dispersion is expected, but this has not been studied in detail. The aim of our study was to investigate the relation between QT parameters and infarct size and left ventricular function in AMI. METARIAL AND METHODS: The study group comprised 57 (52 male / 5 female, mean age; 55 ± 9) patients with first anterior wall AMI. QT dispersion measurements from standard 12-lead electrocardiograms (25 mm/s) were performed at admission before beginning therapy. Echocardiographic studies were performed at day 5. Left ventricular ejection fraction, wall motion score index were calculated. RESULTS: Mean QT dispersion at admission was 54 ±17 ms. There were no significant correlations between QT dispersion and peak creatine phosphokinase levels, left ventricular ejection fraction, and wall motion score indexes ( r =.- 0.010, p =0.9; r =0.190, p = 0.1; r =0.105, p = 0.2, respectively). Patients were divided according to QT dispersion into 2 groups: group 1 (n= 26) with QT dispersion< 50 ms and group 2 (n=31) with QT dispersion > 50 ms. There were no significant differences between two groups in peak creatine phosphokinase levels, left ventricular ejection fraction, and wall motion score indexes (1850 ±1181 vs 1927 + 1627I/U; 45 ±15 vs 48 ±12; 1.6 ±0.3 vs 1.7 ±0.3, respectively, p > 0.05 ). CONCLUSIONS: The findings of our study suggest that QT dispersion on admission electrocardiogram in patients with first anterior wall AMI is unrelated to infarct size estimated by the peak creatine phosphokinase levels and the degree of left ventricular dysfunction.

GEREÇ VE YÖNTEM: Akut anterior miyokard infarktüsü tanısıyla koroner yoğun bakıma kabul edilen, 5 'i kadın 52 'si erkek yaş ortalaması 55 ± 9 olan 57 hasta çalışmaya alındı. Tüm hastalara yoğun bakıma kabulde tedaviye başlamadan önce 25 mm/s hızda 12-derivasyonlu elektrokardiyografi (EKG) çekildi. Çekilen bu EKG'lerden QT dispersiyonu hesaplandı. Beşinci gün hastaların tamamında ekokardiyografık inceleme yapılarak sol ventrikül sistol ve diyastol sonu volümleri, ejeksiyon fraksiyonları, duvar hareket skor indeksleri hesaplandı. BULGULAR: Ortalama QT dispersiyonu 54 ± 17 ms olarak bulundu. QT dispersiyonu ile pik kreatin kinaz düzeyi, sol ventrikül ejeksiyon fraksiyonu ve duvar hareket skor indeksi arasında önemli bir korelasyon saptanmadı (sırasıyla, r = - 0.010, p =0.9; r =0.190, p = 0.1; r =0.105, p = 0.2). Toplam 57 hasta QT dispersiyon sürelerine göre iki gruba ayrıldı. Hastaların 26'sında QT dispersiyonu 50 ms (Grup II) olarak bulundu. Her iki grup arasında pik kreatin kinaz düzeyi, sol ventrikül ejeksiyon fraksiyonu ve duvar hareket skor indeksi açısından anlamlı bir fark yoktu (sırasıyla, 1850 ± 1181 vs 1927 ± 1627I/U; 45 ±15 vs 48 ±12; 1.6 ±0.3 vs 1.7 ±0.3 , p > 0.05 ). SONUÇ: Akut anterior miyokard infarktüsü tanısıyla yoğun bakıma kabul edilen hastalarda kabul EKG'lerindeki QT dispersiyonunun İnfarktüs genişliği ve sol ventrikül sistolik disfonksiyonu ile ilişkisiz olduğu sonucuna vardık.
