Abstract

AIM: QT dispersion has a signifıcant predictive value in the risk assessment of ventricular arrhythmias and sudden death. The preexitation leads to eccentric ventricular depolarization and it results QRS widening in Wolff-Parkinson-White syndrome (WPW). The aim of the study was to evaluate the effects of eccentric depolarization and radiofrequency catheter (RF) ablation on repolarization dispersion in patients with WPW. MATERIAL AND METHODS: The study was performed in 30 manifest WPW patients The mean age was 32±6 years (range 17-50). Twelve leads ECG with 50 mm/sec recording rate were recorded before RF ablation, after l week, and after 5 weeks. QRS duration, QT dispersion (QTd) and JT dispersion (JTd) were measured in these ECG recordings. RESULTS: The mean QRS duration was 119±22 msec (range 80-142 msec), QTd 80+12 msec and JTd was 40+10 msec before RF catheter ablation. There was a signifıcant relationship between the mean QRS duration and QTd and there was no relationship between the mean QRS duration and JTd (r:0.86, r:0.04 respectively) before ablation. There was a signifıcant decrease in the QTd after ablation (46+8 msn, p<0.001). There were no signifıcant difference in the first week and fifth week JTd comparing with the pre-procedural JTd values (41+10 msec, 42+12 msec respectively). As a result there is no relationship between pre-procedural QRS duration and post-procedural JTd, and RF catheter ablation has not effect on JTd values, but QTd significantly changes after ablation. CONCLUSION: Abnormal, eccentric depolarization leads to the prolongation of QRS duration and consequently prolongation of QTd but, has no effect directly on repolarization. The QRS widening and eccentric ventricular depolarization does not cause abnormal repolarisation dispersion in WPW syndrome.
