Abstract
Treatment of a patient with Wolf-Parkinson-White Syndrome Associated with Mitral Stenosis and giant Left Atrial Thrombus Giant left atrial thrombus affecting hemodynamic status is the rare finding in rheumatic mitral stenosis. Atrial dilatation and fibrillation facilitate the thrombus formations. Associating WPW syndrome adversely affect the treatments of these patients. We report the case, 46 years old woman, with hemodynamic deteriations from the giant left atrial thrombus , who went under urgent cardiac operation. In the postoperative period, patient was given amiodarone, so she had sinus rhythm and delta waves disappeared. The successful outcome of this patient demonstrates the effectiveness of the combined surgical and medical treatment.

Romatizma! mitral stenoza bağlı komplikasyonlar arasında hemodinamiyi bozacak derecede büyük sol alriyal trombus nadir görülmektedir. Bu hastalarda gelişen atriyalfibrilasyon ritmi ve atriyal dilatasyon trombus oluşma riskini arttırır. Wolf-Parkinson-White sendromunun eşlik etmesi tedaviyi daha da zorlaştırmaktadır. Olgumuz olan 46 y aşındaki kadın hasta, dev sol atriyal trombüsün neden olduğu hemodinamik bozulmadan dolayı acil cerrahi operasyona verildi. Operasyon sonrası oral antikoagülan tedavi altında amiodarone verilerek medikal kardiyoversiyon yapıldı ve preeksitasyon bulgularının kaybolması sağlandı. Hastamızın kliniğinin verilen tedaviyle düzelmesi, cerrahi operasyonla kombine edilen medikal tedavinin etkinliğini göstermektedir.
